Make a Neuroscience Referral
Thank you for choosing the Neuroscience Center at Valley Children’s as a partner in your patient’s care. We will work hand-in-hand with you to ensure your patient is provided with consistent and comprehensive care.
Refer a patient with our online eReferral system or by fax. Online submissions will receive a confirmation email once the referral is submitted.
About Carelink: Refer a patient via Valley Children's CareLink portal where you will be able to access patient information, follow the progress of care for referred patients and more. This is only an option for those who have a CareLink portal. You can sign up for CareLink here.
Urgent and Emergent Referrals
If your patient must be seen within a week, please indicate in your referral through an urgent flag.
For emergent referrals, please contact the Access Center at 866-353-KIDS (5437) to speak directly with a physician or send the patient to the Emergency Department for immediate treatment.
