    Sports Physicals

    Many school districts require sports clearance before participation in intramural sports beginning in 7th grade. If your child will or might be playing sports within the next year and will need a completed sports physical, please let us know at the time of your child’s well-check. Although there are many features in common between a sports physical and well child exam, they are not interchangeable. If you bring your forms with you or let us know at the time of your well child exam, we can complete the sports physical and well child checkup at the same time. 

    The clearance is good for one year from the time of exam (unless there is a change in health status, e.g. injury). Please have any specific forms your school requires ready at the time of your visit, or you may have to wait 3-4 days for the forms to be completed. 

    Although it is rare, your child or adolescent may need to be referred for further evaluation before clearance can be provided. To minimize disruption in sports participation, we allow at least six weeks between the completion of the physical and the start of the sports season in case your child needs additional evaluation.
